First Day Sample Forms

Greetings from the 6th grade! Our Classroom Management Plan will be sent home with the students on the first day of school to be signed and returned to the homeroom teacher. Also included below are samples of the improvement plan and after-school reflection form for parents to see. Have a great day!

Classroom Management Plan for 6^th Grade

Classroom Rules

1.    Have all appropriate materials/supplies at your desk when the bell rings.

2.    Respect the people, equipment, and furnishings.

3.    Adjust your voice level to suit the activity.

4.    Follow the directions the first time they are given.

Consequences

1^st Time:  Verbal warning

2^nd Time: Walk 10 minutes at recess

3^rd Time: Walk entire recess; Parents contacted via “Improvement Plan”

4^th Time:  Reflection time/room cleaning for 30 minutes after school

5^th Time:  Sent to principal

Severe Disruptions: Student sent immediately to the office

Names and checks will be erased each Friday afternoon.

(Individual student differences will be considered by the teacher in different situations.)

Sample 
"After-School Reflection Time/Room Cleaning" Form

If a student gets a 4th mark during the week, the following form will be sent home in order to inform parents that the child must stay after school for reflection/room cleaning. We hope this makes it more convenient for you to be able to choose a day of the week ahead of time that is best for you to pick-up your child if this occurs. The form must be signed and returned by the date specified, or the amount of time will be extended. Thank you!
